Study goals
Reporting how a supplementary health institution developed a project to restructure the process of inclusion of patients in home care, generating information for the multidisciplinary team, facilitating access to the history of records and communication between the team of evaluators.
Relevance / originality
The relevance of the study is related to the solution found to reduce the time of de-hospitalization of patients and inclusion in the home care service. An original work that enabled the creation of a computerized product in the area of expertise.
Methodology / approach
The Life Cycle Canvas project management visual technique was adopted to obtain a visual activity management framework, using the Trello tool as a process manager: inclusion request, screening, equipment and patient eligibility.
Main results
The product developed with the Trello tool facilitated the team's communication and the recording of information (in application). As a result, lead time was reduced from 97h to 46h, eliminating time on activities that did not add value.
Theoretical / methodological contributions
The study comes to contribute methodologically in face of the scarcity of products or computerized systems that allow for agility in the capture and evaluation of patients for home care.
Social / management contributions
The article helped to facilitate the analysis of information by managers, enabling management at sight (real time) of the phases of the team's work with patients. The main gains were the reduction in the average length of stay in hospitals.
